Are you looking for the latest versions of the Plain Language Disclosures (PLDs)? Do you need to order paper Master Promissory Notes (MPNs) or deferment forms?

The MDHE offers many forms to assist you in administering the federal student loan programs.

Loan Status Update Form – Available for download! This form allows you to report change transactions (e.g., loan maintenance) to the MDHE.

Forms available for order from the MDHE (via Raintree Graphics) – PLDs, all federal deferment forms, MPNs, and other forms may be ordered. (Note: Your school code is your user ID and “mdhe” is your password.)
