MDHE Application for Employment
The Missouri Department of Higher Education has provided an Adobe PDF form (Application for Employment) that you can fill out online. Please be aware that you cannot submit this form online at this time, nor can you save the data you enter; once filled out, you will only be able to print it. Make sure you have access to a working printer prior to filling out this form. To complete the process of submitting this application to the MDHE after filling out the form, you should print it, circle highest year completed under the Education and Training Section on page one, then sign the form on page three under the Applicant's Signature Authorizing to Release Information section.
NOTE: Do not use the Back key on your browser window until you have checked the form, are satisfied with the information you have entered, and have printed it. Once you hit the back key, all information you have entered will be cleared and you will have to fill out the entire form again.
